2)  Must have a current sports physical form on file with the school or turn one into office

      -> The physical must be completed/dated 4/15/19 or later.

Once all the paperwork is turned into the office, the swimmer will receive a confirmation slip from the office. This form must be turned into the coach of the team the swimmer is placed on before she will be allowed to participate in practice. 

Please fill out the Contact Information Form and turn it into Coach Warnica. Contact Forms are due by Wednesday, Nov. 6th.

E-mail is used for EMERGENCY communication ONLY.  This means last minute practice cancellation, meet changes, etc.  All other information will be posted on the website, so check often for updates!
